What Happens on Clinic Day?

When the child/youth and family arrive at the clinic, they will first check in with the Secretary. She will review all of the intake paperwork, will make sure that the insurance on record is up to date, ensure that all releases and authorizations are in place, etc. Also, if there are any missing questionnaires or other needed documentation, she will request that the family/youth complete them at that time. The staff nurse typically then does a current height, weight and blood pressure check on the child/youth.
If it is determined that the child/youth needs to see the entire team
They will work with many entities on the day of clinic. One of the Care Coordinators, typically the Care Coordinator who is assigned to the family, will help facilitate the day for the family. She/He will ensure they see all of the appropriate team members and guide them through their IEPC visit. The child/youth and their family will rotate through seeing an MD or ARNP, a Psychologist, and a Social Worker. After each caregiver has had time with the child/youth and their family, the team gathers for a clinical staffing.

Staffings can be attended by school and community professionals already involved in the child/youth’s care. This helps to ensure that all areas of need are addressed for the child/youth and their family. The staffing time is used to discuss the findings of each team member and converse with the family about possible treatment options to help the youth and/or the family. During this time, the Staff Nurse will also ensure that a follow up appointment is made within the specified amount of time.

If it is determined that the child/youth needs to see a targeted team
They will work specifically with a few of the above mentioned caregivers. This could include any combination of team members. Those who are scheduled to see a child (typically the MD or ARNP) may bring in other team members as deemed appropriate. The Care Coordinator will ensure that a follow up appointment is made within the specified amount of time if that is deemed necessary by the clinical team.
If it is determined that the child/youth needs to be referred to a subspecialist
The Secretary will help compile the appropriate information. She will assist in setting up the appointment in Iowa City, or via telehealth or with a local provider. The Secretary will forward records on to the chosen provider with a written Release of Information. After the appointment, the Care Coordinator or the ARNP will follow up on the recommendations made at the sub specialist appointment and will ensure that a follow up appointment is scheduled if deemed necessary by the treating physician.
